TRUSTED ASSESSOR PROJECT

The Trusted Assessor pilot project will no longer be continuing as from 11th November 2023 due to substantive funding not being made available at this present time.
The aim of the project was to assist in the discharge of patients from Frimley and Wexham hospitals into care homes.
We would like to thank everyone who helped to develop the project and would specifically like to thank our Trusted Assessor, Sue Cole, who has achieved significant successes during her 11 months in the role.
Trust has been built between her and the discharge teams and ward staff within Frimley Park Hospital and with the care homes, residents, and families she has supported. All discharges she has been involved with have been timely and safe resulting in a significant reduction in saved bed days.
The evaluation of the project will include lessons learnt, final data analysis and feedback from services and we hope to be able to share the evaluation in the future particularly with our acute and care home colleagues.
